FDA approves Zilbrysq (zilucoplan) autoinjector as a second self-administration option for adults with generalized myasthenia gravis

UCB announced on October 16, 2026 that FDA approved an autoinjector presentation of Zilbrysq (zilucoplan), a macrocyclic peptide complement C5 inhibitor, for self-administration by adults with anti-AChR antibody positive generalized myasthenia gravis, alongside the existing prefilled syringe.

UCB announced on October 16, 2026 that the U.S. Food and Drug Administration approved a self-administered autoinjector option for ZILBRYSQ (zilucoplan) for adults with generalized myasthenia gravis (gMG) who are anti-acetylcholine receptor (AChR) antibody positive. Zilucoplan is a synthetic macrocyclic peptide that blocks complement component 5, a protein involved in immune-mediated damage at the neuromuscular junction in gMG. Until this approval, the once-daily subcutaneous treatment was available only as a self-administered prefilled syringe; the autoinjector is designed to deliver a pre-defined dose automatically when activated against the skin.

According to the company's announcement, the approval rests on three datasets: DV0012, a phase 1 open-label single-center two-period crossover study in 14 healthy volunteers that established bioequivalence between the autoinjector and the prefilled syringe; DV0013, a phase 3b open-label multicenter study in 31 adults with gMG self-administering by autoinjector once daily, in which complete dose delivery was achieved for 99.8% of 449 returned autoinjector administrations and the most common treatment-emergent adverse event was injection-site pain (3 participants, 9.7%); and a human factors validation study of 100 participants - patients, caregivers, and healthcare professionals - in which 90% completed an injection on the first attempt without training. These are company-reported figures.

UCB said it expects the autoinjector to be commercially available in the first quarter of 2027. The approval adds an administration option; it is not a new indication, and the underlying efficacy and safety profile of zilucoplan in gMG was established in the earlier RAISE program that supported the drug's original approval. The company notes the precise mechanism by which zilucoplan exerts its therapeutic effect in gMG is unknown.

Limits: this brief is based on the manufacturer's announcement; the small supporting studies were open-label and device-focused, and the figures above have not been independently verified here. A device-presentation approval is not new evidence of efficacy, and nothing here is medical advice, dosing guidance, or a source for obtaining the drug. ZILBRYSQ is a prescription medicine for a specific diagnosed population (anti-AChR antibody positive gMG).
